The doctor you visit will only provide free services if he or she is providing so in a hospital. This is the feature of the NHS. It is run by the Department of Health. There are also the private health care providers here as in all other parts of the world.

England has its health services that are primarily run by the NHS. It provides primary and secondary health services. These services are provided by the local NHS organizations. These are looked after by another different group of the NHS trusts.
There are about 300 primary trusts in England. Their job includes strategically planning which looks after the health needs of a particular area. They also make sure that these services are delivered regularly, efficiently and fast. These services include: GPs, dentists, pharmacists, opticians, NHS direct, NHS walk-in centers. It also decides on the kind and quantity of health services to be provided in a certain area. They also take care of patient transports and population screening.
NHS trusts are responsible for running most of the hospitals and health care providers in England. They also ensure that hospitals provide their services efficiently so that the recovery speed is faster and there is fast clearance of backlogs and waiting lists. There are also mental health care services that provide expert assistance in sensitive cases. The health services that are provided in England are fast and regular. You are normally not kept waiting near your phone after calling an ambulance.
The private health care sector is not as big as the NHS. It also does not have the same accountability. It provides GPs, nursing homes, ambulances, hospitals and paramedics. It does not have the responsibility for the health of the wider local community which the NHS shoulders proudly and bravely.
